Life Transition Planning is where your next chapter stops being a vague hope and starts becoming a real, workable plan. Together, we look at where you are today-income, legal needs, recovery, mental health, family ties-and where you want to be in six months, a year, and beyond. Then we break that down into clear goals: steady work, savings targets, repairing relationships, clearing legal hurdles, or moving into your own place.
Each planning session ends with specific action steps, like making a benefits appointment, applying to three jobs, or practicing a new coping skill. We coordinate with your caseworker, counseling, Employment Readiness, and Community Resource Connections so every part of your support system is pulling in the same direction. Over time, you can see your progress on paper, not just in your head, which builds confidence. The benefit is knowing you are not just surviving this season-you are preparing to live independently and on your own terms.
